  1. Basic Policy

    Japan Tissue Engineering Co., Ltd (hereinafter "the Company") shall properly protect all personal information relating to clients, patients, shareholders, directors, staff, etc. (hereinafter "Personal Information"), and in order to handle it appropriately shall set out a basic policy concerning the protection of personal information within the company pursuant to the provisions stipulated in the Law Concerning the Protection of Personal Information as well as to other laws, regulations, guidelines, etc applicable to Personal Information, with which the directors and staff (hereinafter "Members") of the Company shall comply.

  2. Acquisition of Personal Information
    Client information
    The Company may acquire information to the extent necessary to provide its customers with high-quality products and services in relation to its business for the purposes of: accepting all forms of applications, trading contracts for the placement, introduction, development, sales, commission etc. of products, management of customer information, surveys and questionnaires, and sample distribution, including but not limited to the names, addresses, and telephone numbers of clients.
    Patient Information
    The Company may acquire patient codes that enable the identification of specific individuals, through cross-checking with other information held by hospitals, in order to provide products and services to patients with reliability.
    Shareholder Information
    The Company may acquire information to the extent necessary to execute its rights and responsibilities under the Commercial Code, in order to plan facilities for special benefits, etc., or to produce management documentation on the basis of other laws and regulations, including but not limited to the names, addresses, and telephone numbers of shareholders.
    Member Information
    The Company may acquire information to the extent necessary for salary calculations, welfare benefits, education and training, moves and transfers, internal and external publications, hiring decisions, and other personnel and labor reasons, including but not limited to the names, address, telephone numbers, dates of birth, and family composition of members (including prospective employees and others).
  3. Use of Personal Information

    When making use of Personal Information obtained after the purpose has been made clear, the Company shall use this information to the extent necessary to achieve this purpose, and shall handle it properly. When such information is to be used in a way that exceeds the scope of this purpose, or when any change is made to the purpose for which it is used, such use may only take place after authorization has been obtained from the person concerned.
    Personal information that has already been obtained by the Company shall be used properly in conformity with this policy.

  4. Provision of Personal Information to Third Parties

    Except in those cases specifically prescribed by laws and other regulations, the Company shall not provide Personal Information to any third party without obtaining the consent of the person concerned. However, Personal Information may be provided to subcontractors to the extent necessary to achieve the purpose of its use. In this case also, the Company shall require the subcontractor to handle properly the Personal Information provided and shall carry out the appropriate management.
    In addition, in the cases listed below, the Company may be obliged to provide, without the consent of the person concerned, Personal Information to a third party to the extent necessary to achieve the purpose of its use..     2. In cases where such provision is necessary to protect life, body, or financial assets, and there are difficulties in obtaining the consent of the person concerned.

In cases where cooperation with national institutions, regional public bodies, or persons commissioned by them in the performance of their duties as laid down by law is necessary, and there is a risk that obtaining the consent of the person concerned might interfere with the performance of these duties.
  5. Disclosure, Corrections, Additions, Deletions, Cessation of Use, Etc. of Personal Information

    When the Company receives a request from the person concerned for disclosure, correction, addition, deletion or cessation of use of Personal Information, it shall carry out the necessary investigations without delay and respond to this request within a reasonable time period and to an appropriate extent. However, if particular stipulations exist in laws and elsewhere and there are justified reasons for not being able to respond to the request from the person concerned with respect to all or a part of the Personal Information, the Company shall take necessary measures, including but not limited to notification of this reason to the person concerned.

  6. Appropriate Management and Improvement of Personal Information

    The Company shall take appropriate safety measures to prevent improper access to Personal Information as well as loss, destruction, falsification, leakage, etc. of Personal Information. The Company shall also endeavor to carry out awareness raising activities for Members who handle Personal Information, such as implementing education and training for the protection of Personal Information.
    In addition, should the Company receive a complaint relating to Personal Information from the person concerned, it shall respond appropriately and swiftly, and in addition to working to remove the cause, when necessary it shall implement measures to prevent a reoccurrence, supervision and control of Members, and provision of systems.
    The Company shall comply with laws, directives, and all other regulations concerning the protection of Personal Information, and shall strive for continual improvement in the proper handling of Personal Information. Such improvements shall be reflected as necessary in this basic policy.
